• Colorado PERA 401(k) plan

    Today, most financial experts agree that you will need approximately 80 percent of your income to maintain your current way of life in retirement. And while your Colorado PERA Defined Benefit (DB) or Defined Contribution (DC) Plan will contribute to that amount, you may want additional savings.

    The PERAPlus 401(k) Plan is a voluntary retirement savings program offered through PERA. The PERAPlus Enrollment Guide can help you create your plan for the future. 

    1. Complete the PERA 401(k) Contribution Authorization formand send it to HR Connect (Connect_HumanResources@dpsk12.org) (not payroll, Voya, or PERA).
    2. Send the PERA 401(k) Beneficiary Designation Form to VOYA.
    3. After opening a 401(k) account through PERA, you will need to complete the Salary Reduction Agreement to indicate the amount of your paycheck contribution and forward to the Benefits Department for processing or work directly through your representative.

    Please note, the Colorado PERA 401(k) plan is not tied to the Colorado PERA pension plan, but is an additional way to divert a portion of salary toward retirement savings.

    How do I change the amount of my contributions?

    If you’d like to increase or decrease the amount of your contributions, you’ll need to fill out this form: PERA Contribution Revocation Form.

    Send the completed form to HR Connect by emailing Connect_HumanResources@dpsk12.org.

    How do I stop making contributions?

    If you’d like to stop making contributions to an open account, you’ll need to fill out the PERA 401K Revocation Form, and email the completed form to HR Connect (Connect_HumanResources@dpsk12.org).
