Long-Term Care Insurance
DPS offers Long-Term Care (LTC) Insurance through UNUM. LTC insurance is coverage that will pay for assisted living, nursing home care or home health care in the event you are unable to care for yourself because of a chronic condition or disability. Employee premium payments are made through payroll deductions on an after-tax basis. All benefits-eligible employees are qualified to enroll in long-term care insurance.

- To enroll, complete an application on UNUM’s site for DPS.
- To continue coverage after leaving DPS, complete the Election to Continue Your Long Term Care Insurance form and return it to Unum. This form must be completed and returned within 31 days of your benefits coverage end date (for help determining this date, please contact HR Connect at Connect_HumanResources@dpsk12.org or call 720-423-3900.
