Discrimination Prevention & Response (DPR) and Title IX Processes


  • Schedule training! If your team has not had recent, foundational DPR and Title IX training this school year, please contact Kristin to get trained ASAP! Everyone implementing the work must be trained to do so. You can also find recorded modules in the Resource Library (click the tab to the left) and within the Discipline Building Leaders folder managed by Barb Downing. 


    Tell us about your team! Please update this spreadsheet with your current DPR Designee and Title IX Coordinator. 


    Tell your community about your work! Add a section to your website naming your DPR and Title IX work and include contact information for your DPR Designee and Title IX Coordinator, along with any school specific process details, to ensure your students and parents know exactly how to share any Concerns they may have.

    District run schools, select the appropriate report from the menu below. Note, you must be on the DPS network to access these forms. Log into VPN (which allows remote access) if you need to access the forms off-site. If you would like VPN/remote access, please contact DoTS

  • Title IX Summary Form

    Use this form to document Title IX Concerns that did not result in the filing of a formal Complaint form.

    Once you have completed the investigation and Formal Process Report form, please complete the Summary Report Form (one page form) to share with the parents of involved students (redact as needed).

    Informal Resolution Process
    If in the investigation process you determine that an Informal Process is the best course of action (see Informal Resolution Process Guide, if necessary), only the Informal Process Report Form is required.

  • Student Complainant / Employee Respondent

    This section is to be completed by an HR Partner. If you have found yourself in this section and you are not an HR Partner, please contact your HR Partner right away and they will lead the completion of the Student Complainant/Employee Respondent DPR process.

    Once the investigation is complete, the HR Partner must submit the Investigation Summary Report by clicking the button below.

  • Employee Complainant / Student Respondent

    This section provides resources and reporting guidance for incidents with an Employee Complainant and a Student Respondent. This form should be completed by the school DPR designee, with consult from SE&O Safety Coordinator as needed. The HR Partner will support with the complainant/employee side of the investigation. 

    If you are an employee who would like to report discrimination or harassment, notify your HR Partner, or if you would prefer, complete the Complaint Form.

    Once you have completed the investigation and the full report form above, please complete the 
    Employee/Student Discrimination Report Summary form to share with the parents of involved students.

    Informal Process 
    If in the investigation process you determine that an Informal Process is the best course of action (see Informal Resolution Process Guide, if necessary), only the Discrimination Report Summary is required. This is the same form used to provide a final investigation summary if a full report was submitted.

  • Employee Complainant / Employee Respondent

    This section is to be completed by an HR Partner. If you are an employee who would like to report discrimination or harassment, notify your HR Partner, or if you prefer, complete the Complaint Form
