Documentation Requirements

All visits must include a 'proof of visit'.

If visit is done in-person (in-home or at a neutral location), then you can fill out option #1 OR take a selfie with everyone in visit.

If visit is done virtually, then you must fill out option #2.

Proof of Visit Option #1: This form must be signed by the family as 'proof of visit'. Please hold on to your proof of visit in case you are asked to present it during districtwide monthly audit. This half-sheet is for in-person visits.
