  • MetLife Supplemental Life Insurance +

    Full-time DPS employees have the option to purchase MetLife Supplemental Life insurance for yourself, your spouse and your dependent children, in addition to the Basic Life and Automatic Death & Dismemberment (AD&D) insurance which is automatically provided at not cost to benefits-eligible employees. You must purchase voluntary coverage for yourself in order to purchase coverage for your spouse and/or dependents. View the full list of rates.

    Grief Counseling services are provided by LifeWorks US Inc. and are included as part of the MetLife Basic and Optional Life products. For additional information, review the MetLife Grief Counseling Information Sheet.

    Will Preparation, with in-person assistance, is also provided complimentary with optional life insurance through Willscenter and is an online service.

  • MetLife Basic Life and Automatic Death & Dismemberment (AD&D) Insurance +

    DPS automatically provides Basic Life and Automatic Death & Dismemberment (AD&D) insurance to all active, benefits-eligible employees at no cost through MetLife at the rates below:

    • Full-time employee Basic Life Coverage: 2x your annual earnings, a maximum of $300,000
    • Full-time employee AD&D coverage: 2x annual earnings, a maximum of $300,000
    • Part-time benefits-eligible employee Basic Life coverage: $2,500

    *Coverage amounts begin to reduce at age 65.

    To continue this insurance while on unpaid non-FMLA leave, complete the MetLife Waiver.

  • Colorado PERA Optional Life Insurance +

    All DPS employees have the option to purchase Colorado PERA Optional Life insurance for yourself, your spouse and your dependent children, in addition to the Basic Life and Automatic Death & Dismemberment (AD&D) insurance which is automatically provided at not cost to benefits-eligible employees.  PERA offers an optional group decreasing term life insurance plan administered by UNUM called Plan 1. In a decreasing term life insurance plan, the cost remains the same as you age while the benefit amount decreases. You can enroll online by logging into your PERA account using your User ID and password, or you can print this form and mail it to Unum.
