Well Aware Program

  • Employees who have elected DPS benefits have the opportunity to earn $200 after completing 2 steps: (1) completing the online health risk assessment through their chosen DPS medical insurance, steps here, and (2) by completing an annual physical through their physician and follow the recommendations from their doctor based on age, gender, family history and medical history. 

    • For employees enrolled in a Consumer Driven Health Plan, $200 will be contributed to your Health Savings Account.
    • For employees enrolled in an HMO Plan, $200 will be awarded to your paycheck.

    The deadline to complete the Well Aware program for the school year 2023 - 2024 in May 18, 2024.
