Well Aware Program

  • The Well Aware Program is an incentive-based wellness program through DPS.  This program encourages employees, who have elected DPS benefits, to complete both the online health risk assessment and age-based preventative screenings to earn a $200 stipend

    • For employees enrolled in a Consumer Driven Health Plan, $200 will be contributed to your Health Savings Account.
    • For employees enrolled in a HMO Plan, $200 will be awarded to your paycheck.

  • + Is it safe to complete my preventative screenings in-person due to COVID?

    Aetna - Members are encouraged to complete their preventative screenings, but first, an individual should contact their provider’s office to determine the best course of action, as this may be different for each member depending upon each member’s health journey.

    Kaiser Permanente - In response to COVID-19, Kaiser Permanente recommends delaying biometrics and cancer screenings that need to be completed for your Well Aware program.  However, if you have questions, we encourage you to connect with your KP care team to discuss your personal situation.  There are a number of ways you can connect with your care team from the safety of your home.  On kp.org, you can schedule a video or phone visit with your physician or securely email your doctor.

  • + How do I navigate to my employee portal to start the Health Risk Assessment?

    Below are directions for how to navigate to the assessment based upon your healthcare provider:

    Aetna steps for finding the Health Risk Assessment

    •       Log into your Aetna secure member website – Aetna.com
    •       Once logged in, click on “Stay Healthy” at the top of the page
    •       Click on “Stay Healthy” again to access the wellness webpage.
    •       Click on “Complete a Health Assessment”. This will redirect you to a new webpage where you can begin the assessment.  

    Kaiser steps for finding the Health Risk Assessment

    •       Visit the Kaiser Permanente landing page
    •       Sign on using your Kaiser username and password
    •       Visit DPS Rewards Welcome page and scroll down to complete the Total Health Assessment
    •       *It can take up to 24-48 hours for the employee portal to show a green check mark and “done”, indicating you’ve completed the health risk assessment.

  • + How can I check that I've successfully completed the online Health Risk Assessment?

    Unfortunately, the Employee Wellness team does not have access to your personal benefit portal and cannot check the status of its completion at this time. Please either visit your benefit portal and follow the steps below or contact Kaiser and Aetna directly to check the status of your assessment.

    Aetna steps for finding the status of your Health Risk Assessment

    • Log into your Aetna secure member website – Aetna.com
    • Once logged in, click on “Stay Healthy” at the top of the page
    • Click on “Stay Healthy” again to access the wellness webpage.
    • Click on “Complete a Health Assessment”. This will redirect you to a new webpage. 
    • The completed date will be listed below the Health Assessment button.
    • To check that you have completed all of the requirements, you can also contact the number on the back of your Aetna benefit card.  

    Kaiser steps for finding the status of your Health Risk Assessment

    • Visit the Kaiser Permanente landing page
    • Sign on using your Kaiser username and password
    • Visit DPS Rewards Welcome page and scroll down to the section about the Total Health Assessment.
    • If completed, you will see a green check mark and “done”.
    • *Please allow the system 24-48hrs to register that you’ve completed the Health Risk Assessment portion on the Kaiser Permanente site.

  • + When will I see my $200 stipend?

    The stipend can take up to 6 weeks to be processed for payroll once you've completed both the Health Risk Assessment and your preventative screenings.

    • For employees enrolled in a Consumer Driven Health Plan, $200 will be contributed to your Health Savings Account (HSA).
    • For employees enrolled in a HMO Plan, $200 will be awarded to your paycheck.
