Well Aware Program

  • Employees who have elected DPS benefits have the opportunity to earn $200 after completing 2 steps: (1) completing the online health risk assessment through their chosen DPS medical insurance, steps here, and (2) by completing an annual physical through their physician and follow the recommendations from their doctor based on age, gender, family history and medical history. 

    • For employees enrolled in a Consumer Driven Health Plan, $200 will be contributed to your Health Savings Account.
    • For employees enrolled in an HMO Plan, $200 will be awarded to your paycheck.

    The deadline to complete the Well Aware program for the school year 2023 - 2024 in May 18, 2024.

  • + How do I navigate my employee portal to start the online Health Assessment?

    Below are directions for how to navigate to the assessment based upon your healthcare provider:  

    Kaiser steps for finding the Health Assessment

    •       Visit the Kaiser Permanente landing page
    •       Sign on using your Kaiser username and password
    •       Visit DPS Rewards Welcome page and scroll down to the Health Assessment portion to begin the process 
    •       *It can take up to 24-48 hours for the employee portal to show a green checkmark and “done”, indicating you’ve completed the health risk assessment.

    MotivHealth steps for finding the Health Assessment

    • Log into your MotivHealth secure member website MotivHealth.com
    • Once logged in, you will be introduced to the Well Aware Program and can begin the Health Risk Assessment

  • + How can I check on the status of my program requirements?

    Unfortunately, the Employee Wellness team does not have access to your personal benefit portal and cannot check or change the status of its completion at this time. Please either visit your benefits portal and follow the steps below or contact Kaiser and Aetna directly to check the status of your program.

    Kaiser steps for finding the status of your Health Risk Assessment and program requirements

    • Visit the Kaiser Permanente landing page
    • Sign on using your Kaiser username and password
    • Visit DPS Rewards Welcome page and scroll down to the Health Risk Assessment section
    • If completed, you will see a green checkmark and the word “done”
    • *Please allow the system 24-48hrs to register that you’ve completed the Health Risk Assessment portion on the Kaiser Permanente site

  • + When will I see my $200 stipend?

    The stipend can take up to 14 weeks to be processed once you've completed both the Health Risk Assessment and your preventative screenings.

    • For employees enrolled in a Consumer Driven Health Plan, $200 will be contributed to your Health Savings Account (HSA).
    • For employees enrolled in a HMO Plan, $200 will be awarded to your paycheck.
