Well Aware Program

  • The Well Aware Program is an incentive-based wellness program through DPS.  This program encourages employees, who have elected DPS benefits, to complete both the online health risk assessment and age-based preventative (Well Adult/Well Woman) screenings to earn a $200 stipend

    • For employees enrolled in a Consumer Driven Health Plan, $200 will be contributed to your Health Savings Account.
    • For employees enrolled in an HMO Plan, $200 will be awarded to your paycheck.

  • + How do I navigate my employee portal to start the online Health Assessment?

    Below are directions for how to navigate to the assessment based upon your healthcare provider:

    Aetna steps for finding the Health Assessment

    •       Log into your Aetna secure member website – Aetna.com
    •       Once logged in, click here and follow these steps to complete the Health Assessment.    

    Kaiser steps for finding the Health Assessment

    •       Visit the Kaiser Permanente landing page
    •       Sign on using your Kaiser username and password
    •       Visit DPS Rewards Welcome page and scroll down to the Health Assessment portion to begin the process 
    •       *It can take up to 24-48 hours for the employee portal to show a green checkmark and “done”, indicating you’ve completed the health risk assessment.

  • + How can I check on the status of my program requirements?

    Unfortunately, the Employee Wellness team does not have access to your personal benefit portal and cannot check or change the status of its completion at this time. Please either visit your benefits portal and follow the steps below or contact Kaiser and Aetna directly to check the status of your program.

    Aetna steps for finding the status of your Total Health Assessment and program requirements

    • Log into your Aetna secure member website – Aetna.com
    • Once logged in, click on the “Health & Wellness” tab at the top of the page
    • Click on the “Stay Healthy” button again to access the wellness webpage
    • Click on the “Incentives” tab, which will direct you to the DPS Well Aware Program incentive
    • This tab will allow you to see what you've completed within the Well Aware Program this year
    • If you have questions about meeting the requirements, please call the DPS concierges at 855-220-6416 or use the number on the back of your Aetna benefit card

    Kaiser steps for finding the status of your Health Risk Assessment and program requirements

    • Visit the Kaiser Permanente landing page
    • Sign on using your Kaiser username and password
    • Visit DPS Rewards Welcome page and scroll down to the Health Risk Assessment section
    • If completed, you will see a green checkmark and the word “done”
    • *Please allow the system 24-48hrs to register that you’ve completed the Health Risk Assessment portion on the Kaiser Permanente site

  • + When will I see my $200 stipend?

    The stipend can take up to 6 weeks to be processed once you've completed both the Health Risk Assessment and your preventative screenings.

    When your reward has been processed, you will receive an email notifying you of the date it will be deposited into your HSA or added to your paycheck.

    • For employees enrolled in a Consumer Driven Health Plan, $200 will be contributed to your Health Savings Account (HSA).

  • + Does Aetna still require a paper form?

    No -- The paper form is not required this year. Please log in to your Aetna Wellness portal to begin the process.

    Kaiser Permanente - In response to COVID-19, Kaiser Permanente continues to take all necessary precautions to protect the health and safety of its members. They've expanded virtual and phone care options and encourage individuals to receive their preventative screenings, but ask that you contact your provider's office first to discuss the best course of action.
