This statement will help you understand your benefits and the established value of your total rewards.  The statement is intended to provide an overview of each benefit and the estimated dollar value of those benefits. 

While efforts have been made to ensure the accuracy of this statement, the possibility of errors exists.  In the case of a discrepancy between the statement and the benefits plan statement, the terms set forth in the benefit plan document will govern.
