One of the most important duties faced by every school office is the ongoing update of household data for students within the District.  A key piece is ensuring that a correct current residence address is associated with each student.

 If you are unable to locate an address in the Infinite Campus Address File, you need to click here on the online ADDRESS REQUEST FORM. 

1. DQMT School Request Summary  link at the top of the form will display the address requests you have submitted for the year, including status.  Select your school from the drop down.  The requests are sorted with the most recent at the top.
2. Follow the instructions for entering your information
3. Click Submit
4. The form will let you know if you have forgotten anything important (i.e. A street number is required)
5. When your submission is complete, this message will appear Your request has been submitted, DQMT will contact you with any questions.  Please note that if you do not receive this message, your request will not be processed.
6. Once you have successfully submitted your form, your name, ext and school will be populated and ready to enter your next request. 
7. You will receive an email response from a DQMT representative within 48 hours, either confirming that the address has been created or requesting further detail.
