Submitting Your Proof of COVID-19 Vaccination (Posted 9.2.21)

RE:  Proof of COVID-19 Vaccination

 

All DPS employees must be fully vaccinated against COVID-19 by September 30th. You must prove you’re vaccinated by submitting a copy of your vaccination card to HR. You do not need to give it to your supervisor. HR is keeping the information in a confidential file, so no one else will be able to see your status, just those HR employees who are verifying what you provide.

 

If you’re having trouble uploading a copy of your vaccination card, there will be tech support in the terminals later this month. 

 

Hilltop Terminal: On Tuesday, September 14th, you can come to the Hilltop Terminal for help from 8:30 A.M. to 12:00 P.M. Make sure you have your COVID-19 vaccination card, and then head to the Admin Conference room for help between 8:30 A.M. and noon on Tuesday, September 14th.

 

Northeast Terminal: On Thursday, September 16th, you can come to the Northeast Terminal for help from 8:30 A.M. to 12:00 P.M. Make sure you have your COVID-19 vaccination card, and then head to the Training Room for help between 8:30 A.M. and noon on Thursday, September 16th.

 

If you’d rather upload a copy of your card yourself, you are more than welcome to do so. Please follow the instructions below to upload a copy of your vaccination card.

 

INSTRUCTIONS FOR UPLOADING A COPY OF YOUR COVID-19 VACCINATION CARD

Log in to Employee Space and follow the below steps to upload your vaccination compliance or apply for a medical or religious exemption. 

  •  Submit Documentation of Vaccine Completion: Upload a picture of your completed vaccination card using these instructions. Note: Only JPEG and PDF file types are allowed in the system.
  • Lost your vaccine card? Click here to learn how to obtain your COVID-19 vaccine record. If you got your vaccination in Colorado, you can also download the myColorado App and click on the myVaccine Record section. The app may require additional log-in information to access the record the first time.
  • Submit a request for a medical exemption: You and your medical provider will need to complete this form prior to uploading to Employee Space. Follow these instructions to upload your documentation.
  • Submit a request for a religious exemption:  Complete this form to request religious exemption. Please note that all sections of the form must be completed, including a description of the sincerely held belief. Follow these instructions to upload your documentation. 

Medical accommodation documentation will be reviewed and approved by the Leave of Absence team. Religious exemption documentation will be reviewed and approved by the Employee Relations team. Approved requests will be shared with HR Partners and your manager. Your manager will not have access to your private information. Managers are responsible for determining appropriate steps for accommodation. Examples may include masking, regular testing, or other actions to ensure the safety and health of yourself, DPS students, and other staff.

If you are interested in receiving the vaccine, it is free and available to anyone in Colorado 12 years old and older. To get your vaccine, you can visit Denver Health clinics on a walk-in basis, or visit the State of Colorado’s vaccine website for more locations.
