RE: Gauging Interest in On-Site COVID-19 Vaccinations
We would like to help interested Transportation employees receive the COVID-19 vaccine. In order to help employees get vaccinated, we are considering hosting a vaccination clinic at the terminals. Before we arrange the clinic, we would first like to gauge interest. If you are not already vaccinated, please take this anonymous survey letting us know if you would like to receive your COVID-19 vaccination at your terminal. The survey isn’t taking any identifiable information about you, so please feel free to be honest when answering.
