RE:  Proof of COVID-19 Vaccination

 

If you are having trouble uploading your COVID-19 vaccination card, we are here to help.

 

Hilltop Terminal: Please visit Tyler Maybee’s office for help. To set up a time to come to his office, please call him at 720-423-4634 or email him at tyler_maybee@dpsk12.net.

 

Northeast Terminal: Tyler will be in Northeast Terminal’s training room from 8:30 A.M. to 12:30 P.M. on Thursday, October 28th. You can stop by the training room during that time window for help.

 

If you’d rather upload a copy of your card yourself, you are more than welcome to do so. Please follow the instructions below to upload a copy of your vaccination card.

 

INSTRUCTIONS FOR UPLOADING A COPY OF YOUR COVID-19 VACCINATION CARD

Log in to Employee Space and follow the below steps to upload your vaccination compliance or apply for a medical or religious exemption. 

  •  Submit Documentation of Vaccine Completion: Upload a picture of your completed vaccination card using these instructions. Note: Only JPEG and PDF file types are allowed in the system.
  • Lost your vaccine card? Click here to learn how to obtain your COVID-19 vaccine record. If you got your vaccination in Colorado, you can also download the myColorado App and click on the myVaccine Record section. The app may require additional log-in information to access the record the first time.
